Overview
The Record
Saphren arranges medicine around a clear lake rather than confining it to sealed clinical towers. Hospitals, physician academies, laboratories, botanical conservatories, rehabilitation residences, gardens, and water courts extend through the surrounding hills. Low-gravity transit joins them into a city where treatment may begin in a ward, continue through long residence, and rely on the cultivated environment between the two.
Medicinal materials and biological knowledge arrive from Myr and from Sorell’s protected growers under strict controls. Saphren studies, prepares, and teaches their use while receiving patients and physicians from powers that may agree on little else. Its schools also conduct the only formal examination of Menders in the system. The examination is public and certifies a witnessed record of work, not an assertion about what a practitioner might be capable of.
Each hospital, school, and research institution directs its own work under an Ilyran charter and must publish the reasoning by which it acts. The exception is the patient’s confidence. Saphren’s authority depends on holding both obligations at once: medical knowledge must be testable and available, while recovery still requires rooms in which a person is not made public.

Governance
Medical, educational, and civic institutions administer Saphren under public charters granted at Elaris. Each institution directs the work named in its charter and must publish its methods and decisions. Patient confidence is the standing exception: what is disclosed to a physician does not enter the public record.
Environment and Travel
Environment
Hospitals, research halls, botanical galleries, recovery residences, therapeutic gardens, and water courts surround a clear shallow lake and extend into cultivated hills. Low-gravity transit connects the institutions without separating treatment from the landscape in which patients recover.
